We understand that experiencing issues with tags or applicators can be frustrating. We’re here to help. By submitting a completed complaint form and, when possible, providing samples of the affected indicators, you will help us to investigate the issue thoroughly and work toward a resolution.

Please ensure that this form is fully completed for each individual complaint related to indicators. Accurate and complete information is essential to support effective review and follow-up. If possible, include physical samples of the problem indicators associated with the complaint.

All completed forms and any accompanying indicator samples should be sent to the Canadian Cattle Identification Agency (CCIA) Client Support Team for evaluation. Materials should be mailed to the following address:
